Step 4: Formal investigation
Carry out a formal investigation to gather details of the alleged misconduct.
Make all reasonable efforts to establish and confirm the facts by:
- Interviewing all persons who may have information.
- Obtaining all relevant information and documents.
- Questioning any witnesses about what they saw.
- Reviewing the witness statements and corroborating their statements and challenging any apparent inconsistencies.
- Keeping comprehensive and accurate records relating to the investigation.
- Carrying out the investigation in an impartial, unbiased and open minded manner.
Note: Present all information obtained in the investigation to the employee so they have the opportunity to consider it before being interviewed.
